KMouth is probably the closest thing I can think of. Dunno if it takes PDF files, which are a whole different beast than ASCII/UTF-8 text files.

Sioyek seems pretty good if you read lots of papers and like using multiple highlight colors etc.! Check out its tutorial:

I’ll try to compile it on FreeBSD.

PS: It has an extension system. Adding a text to speech extension should be investigated.
